How to Apply for an Immunization Exemption in the State of Arkansas:

  1. Request the packet directly from the Arkansas Department of Health at : 501-661-2169 or 501-661-2169
  2. Complete paperwork and return to the AR Department of Health per their instructions, after the forms are notarized.        
  3. Please allow 2-4 weeks for processing of the application.
  4. Remember that all exemptions must be renewed at the beginning of each school year.
  5. Once the exemption letter is received, you must deliver a copy to the building nurse. This letter will only be sent to parent.
